Does your child have any medical conditions that we should be aware of?

Photo Permission : From time to time we take pictures during Jumparoo activities to send home as keep sakes or for use in your centre or school. Jumparoo would like your permission to use pictures taken in our classes for promotional materials. This may include our Facebook page, Newsletters and Calendars. We will never reference your child by full name (first name only) or provide any specific information regarding your child. We also will never sell these pictures; we will use them exclusively for Jumparoo purposes only.

By clicking “I agree" below, I authorise my child to attend Jumparoo Classes. I acknowledge Jumparoo uses its best endeavours to ensure that all classes and equipment are safe for children. All physical activities possess inherent risks, I understand and accept the associated risks. I acknowledge that I am the parent/guardian of the child listed at the top of this form. I have read and agreed to the enrolment terms and conditions.

Credit Card Details

Jumparoo uses Secure Payments Gateway for all transactions. It is the responsibility of the parent to contact Jumparoo before the debit takes place to charge credit card or bank account details. A $5 dishonour fee will be charged for for insufficient funds / expired credit cards.
